Centers for Medicare & Medicaid Services (CMS) provides a search tool for the Medicare Physician Fee Schedule (MPFS). Physicians can search this database of fees to find current rates for CCM/CPO in their geographic area. National rates are also available for reference.
The Chronic Conditions Data Warehouse provides access to national CMS Medicare and Medicaid research data. Condition category variables sorted into claims-based algorithms are used to determine if treatment appears to have taken place. All variables are currently available in the Master Beneficiary Summary File (MBSF), Medicare-Medicaid Linked Enrollee Analytic Data Source (MMLEADS), and Medicaid Enrollee Supplemental File (MESF).

These answers apply to physicians and nonphysician practitioners such as Certified Nurse Midwives, Clinical Nurse Specialists, Nurse Practitioners, Physician Assistants, or any other provider who bills the Medicare fee-for-service program (Original Medicare).
