PROSPECTIVE HEALTH ASSESSMENTS
Advantmed’s Prospective Health Assessments provide actionable 360-degree insights into the health status of an organization’s member population. Prospective Health Assessments focus on addressing new conditions, re-capturing and documenting previously unrecorded chronic conditions to close quality care gaps and drive risk adjustment improvements. Prospective Health Assessments can occur at any time during the year and can be scheduled for Medicare Advantage, Commercial and Medicaid members that have been non-adherent in scheduling primary care appointments.
Members have the ﬂexibility of having the assessment conducted by Advantmed’s highly-trained nurse practitioners in their home, in a centralized Advantmed clinic or in a primary care physician’s ofﬁce.
